Sign Up Online and Print Forms
Apply online for Pack 464 on Scouting.org.
Complete your Pack 464 Registration form and send to pack464wc@gmail.com.
Submit your Family Talent Survey (coming soon).
Complete the Activity Consent Form.
Sign the Photo Release Form.
Complete Health Form Part A and B of the BSA Annual Health & Medical Record*.
*Note Pack 464 only requires parts A & B. We do not have any activities scheduled that will exceed 72 hours, which makes part "C" unnecessary. For more help, see instructions here or learn more about annual health and medical requirements.
